If you are residing in the United States and are or where a patient of Southern Illinois Healthcare Enterprises, Inc., Southern Illinois Hospital Services, and/or Southern Illinois Medical Services, NFP and signed into or scheduled appointments through Defendants’ MyChart patient portal or website open scheduling from February 21, 2022, to February 28, 2023, or June 12, 2023, to July 6, 2023 or completed a health risk assessment from November 2, 2020, to January 31, 2021 or February 1, 2021, to April 30, 2021, you may be a Class Member and eligible for payment. Excluded from the Settlement Class are: (1) the Judge in this case, and the Judge’s family and staff; (2) Southern Illinois and its officers, directors, and related companies; (3) anyone who validly excludes themselves from the Settlement; and (4) attorneys for the parties.
If you are a Class Member and wish to receive a cash payment then you must submit a claim form by June 15, 2026.
Southern Illinois has agreed to pay for two benefits that are available to all Class Members.
CyEx Privacy Shield Pro. All Class Members are eligible to enroll in one (1) year of CyEx Privacy Shield Pro. Enrollment codes have been sent to all Class Members, and you will receive activation instructions after the Settlement has received final approval. If you no longer have your enrollment code, please contact the Settlement Administrator. Cash Payment. All Class Members may claim a one-time $17.50 cash payment. You do not have to provide any proof or explanation to claim this payment.
To start your online claim form, you must login with your Unique ID and PIN. You can find your ID and PIN on the Notice that was sent to you. The deadline to submit a Claim Form online is June 15, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than June 15, 2026.
